Wow, that is horrible for an xB. I would try running injector cleaner through it and take it in for a full tune up. Also, don't hammer on it from stop lights. Gradually accelerate. Not sure why you are getting such low MPG's other than that.

You need a tune up. You shouldn't in any way get less than 28mpg.
Definitely look at the plugs and filter.

Something is really wrong , my B gets 30 most all the time and the last trip I had I got 35 to 38 in 600 miles ,gotta keep your foot out of it .

Is it a stick or an automatic? If it is a stick, try shifting right around 3000 rpm's. And like everyone else said, keep your foot out of it.

Also...are you running stock suspension, wheels and tires on this? A tune up is good but having a somewhat tricked out suspension will usually lower fuel economy. This is typical of having wide tires and heavy rims. It takes more to spin them because they're heavier.
Another thing, check your tire pressure. It's an easy and often times overlooked gas sucker.
Tune up is easy on the box. Four plugs and an air filter and it's golden.

Angela's right...5lbs a tire will really hurt your MPG's. It's almost a 55 miles per tank difference in mine.
